For your convenience, we have HIPPA and Patient History forms available to download. This allows you to print the forms and complete them in the comfort of your home. When completed, just bring the forms with you to your scheduled eye exam.
It would be helpful to the Doctor if you bring a list of your medications and your current medical insurance cards.
